Kevin is a dedicated Population Health Specialist with a focus on community engagement centered around disease prevention, through health education and strategic maneuvers leveraging community relationships. Kevin has a knack for maintaining compliance and quality assurance. Kevin’s ultimate goal is to support high risk patients in reducing their risk of hospitalizations. Kevin looks to achieve this goal via patient empowerment and developing systems that value all care team member experiences.
